Policies, Fees, and Payment

Information about Fees and Payment:

Payment is required at the time of service.  For specialty services (ART consultations, Gamete Donor Evaluations, Gestational Surrogacy Evaluations), payment will be required in full prior to meeting with Dr. Riddle.  For Collaborative Law Agreements, a retainer will be due at the time of the first meeting with Dr. Riddle.  At the conclusion of the collaborative process, you will be refunded any money that is left over.

I am a participating provider with several insurance companies.  If I am in network with your provider, I will bill the insurance company on your behalf.  You are responsible for any co-pays at the time of service.  If your insurance requires prior authorization, you will need to provide that information at the time of your first visit.  If I am not in network with your provider, you are responsible for paying my fee in full at the time of service.  You may be eligible for partial reimbursement from your insurance carrier, but you will be responsible for following up with your carrier to see if you are eligible for out-of-network benefits.

When new patients schedule, you will be asked to sign a notice of privacy practices, a consent for treatment, and a document outlining practice policies.
